Flu Shot Registration (Community Celebration)

We're offering free flu vaccines to the community during our Community Trunk or Treat event on Sunday, October 15, 2023 from 3 to 5 pm. To register, please fill out the form below and select your preferred appointment time. We cannot offer flu shots to patients 65+ during the event.
